Vital Records
The fee for a vital record is $15.00 for the first copy and $6.00 for each additional copy purchased at the same time. No Out of State checks accepted, so send a certified bank check or money order.
All applicants must provide proof of identity, which can be a copy of their Driver's License or State ID. Documents that do not have your name on it will require providing proof of lineage that can include your birth certificate, court document, military record, hospital record, physician's record, newspaper clipping, etc.
